Van Ackeren Consulting, LLC DBA Personalized Prevention was founded in 2006, it is a privately owned, domestic limited liability company that began as a strategic consulting firm for employers and has grown to specialize in population health management, wellness and prevention. The Principals of Personalized Prevention have spent over two decades in the healthcare and insurance industries and are dedicated to their mission of reducing insurance costs for employers.

Personalized Prevention offers proactive care management, wellness and preventive programming to individuals and their families through employers associations and claim payers.  Without compromise, Personalized Prevention provides all the essential elements of a successful population health management program- Assessment, Intervention, Education, Access and Incentives. Our solutions are flexible. We manage and coordinate a personalized combination of programs that target the specific population needs of each client. Our Personalized Prevention programs are designed to reduce risk factors and lead populations to better overall health and well being.

Our programs can be built slowly to build an employers internal program over time, or we can implement our comprehensive solution immediately.

We will create a seamless experience for individuals within an employer population and we will simplify the administration of health and wellness programming for employers by serving as a single point of contact for account management, execution and reporting.

The Mission of Personalized Prevention is to reduce insurance costs for employers with solutions intended to prevent future costs.

We aim to be recognized as the nations leading population health management firm and deliverer of comprehensive risk management programs and service. Bundled or un-bundled Personalized Prevention will customize each program to meet the unique needs of each client by bringing together the broadest and highest quality of specialized services to help employers build world-class insurance programs. Our approach is to manage for employers programs aimed at reducing current expenditures with solutions intended to prevent future costs.

Our Model

Our solutions are modular in design. We provide a personalized combination of programs that target specific population needs and that coordinate with existing internal programs and resources. We will be the only provider that is able to coordinate and manage individuals in numerous ways based on each individuals personal preferences. Our interactions accommodate each individuals preferred mode of communication, cultural background, readiness for change, overall health status, and specific barriers to better health. Depending on the need, we will deliver our best in class interventions by phone, web, mail and IN PERSON. This modular approach helps the healthy population maintain a successful path, the at-risk population to be empowered to better manage their health, and the chronic population to be equipped to better manage their health conditions. We deliver the right touch points to the right person at the right time, resulting in Personalized Prevention for individuals and a measurable return on investment for employers. Description of Services

Personalized Prevention uses a 6-step process for maximizing the success of our Personalized Prevention model.

1. DISCOVER: This initial phase is an in-depth review of an employers population, culture, environment, communication channels and resources. Information collected in the discovery process will be used to outline a tailored engagement strategy.

2. PROMOTE: The promotion phase focuses on Incentives/ Disincentives, Communication, and Intelligent Recruitment. We have custom incentive strategies to meet your company’s needs, wellness goals, appeal to your employees and are results oriented. Our on-site promotion in addition, sets the stage for the development and launch of a strategic communication/promotional campaign. Intelligent Recruitment leverages user data from the Health Risk Assessment and delivers targeted promotional emails to drive participants toward the proper intervention. We also offer automated phone based outreach offering a personalized phone call to guide targeted participants to the proper intervention.

3. ENGAGE & INCENTIVIZE: Personalized Prevention drives a deeper level of engagement in our programs by aligning the proper incentives and engaging individuals in numerous ways, including leadership training, on-site promotions, targeted messaging, telephonic outreach, text messaging, web-based health coaching and even face-to-face health coaching.

4. ASSESS: Accurately identifying health needs at the population and individual level is key to driving success in health improvement initiatives.  Personalized Prevention will utilize medical and pharmacy claims data, combining health risk assessment with biometric exam to determine health risks among individuals.

5. INTERVENE, EDUCATE & MOTIVATE: Personalized Prevention provides support to encourage individuals to achieve their greatest potential no matter where they lie on the health care continuum. Our trained health coaches, disease management nurses, dietitians and pharmacists employ leading technologies and behavioral science techniques to inspire change within the populations we serve. Our integrated technology will allow individuals to track their progress and allow the health coaches and providers to see real time results to better manage the individuals they serve. Our on-site presence is instrumental in building rapport, and placing accountability on the individuals.

6. MEASURE & REPORT: Measurement of our program results is essential for three main reasons. First, it enables us to quantify the value we create for our clients. Secondly, it allows us to modify our strategies throughout the program to what works best in each employer population. And finally measurement and reporting allows us to track and trend the population (HIPAA compliant) to identify and recommend programming as well as measure outcomes specific to each population.
